The Institute for Basic and Applied Research\u00a0in Surgery (IBARS) provides additional funding support to\u00a0the Department of Surgery at the University of Minnesota\u00a0with an emphasis on Solid Organ Transplantation,\u00a0Surgical Oncology and Cardio-Vascular Surgery.<\/em><\/p>\n<p>As with Dental School, most physicians graduating from\u00a0Medical School have a total debt load of approximately\u00a0$300,000.<\/p>\n<p>Three \u201ccataclysmic financial events\u201d occur when many students\u00a0graduate from Medical School. About one third marry another\u00a0medical student, effectively <span style=\"text-decoration: underline;\">doubling<\/span> their tuition debt load.\u00a0Next, Medical Students tend to buy homes they cannot\u00a0realistically afford. And third, it is not at all uncommon for\u00a0Medical Students to have a baby, effectively taking one of the\u00a0wage earners out of the full-time practice of medicine.<\/p>\n<p>Painfully, healthcare economists have calculated if a medical\u00a0school couple assumes a \u201cnon-dischargeable\u201d debt load of\u00a0approximately $1 million dollars, over the course of a 42 year\u00a0professional career, there is \u201cgreat likelihood\u201d they will not be\u00a0able to retire their tuition obligation.<\/p>\n<p>The average income of a Mpls.\/St. Paul Family Practitioner is\u00a0$177,600, but new grads often earn less than $100,000\/year in\u00a0their first 3 years.<strong>*<\/strong><\/p>\n<p>This has resulted in the non-too-subtle self-segregation that is\u00a0taking place between the ranks of the primary care physicians and\u00a0the specialists. Recognizing the extraordinary costs involved,\u00a0many students are gravitating to higher paying sub-specialty\u00a0disciplines which, in turn, render the relative salary structure of\u00a0the general practitioner to disproportionately fall behind.<\/p>\n<p><span style=\"text-decoration: underline;\">THE 2012 AFFORDABLE CARE ACT (ACA)<\/span><\/p>\n<p>Economists, political strategists, and business leaders have\u00a0argued for years to expand the base of the insurance pyramid.\u00a0Enrolling young, healthy candidates would help offset the\u00a0exploding costs of health care costs today.<\/p>\n<p>At the national level, this desired-for goal has not played out. In\u00a0fact, young people remain reluctant to sign up for healthcare\u00a0insurance. Even with a bitter pill of an annual Income Tax\u00a0penalty assessed to those without healthcare insurance, many\u00a0argue it is simply cheaper to \u201cpay the penalty.\u201d What the nation\u00a0has witnessed over the past two years is a disproportionate\u00a0enrollment among young and middle-aged participants whose\u00a0needs for healthcare are defined by a host of chronic medical\u00a0problems.<\/p>\n<p>While some states have done an aggressive job of moving many\u00a0off of the Medicaid roles by offering healthcare insurance\u00a0subsidies, the sad fact remains that too often young people today\u00a0suffer a disproportionate number of problems related to obesity,\u00a0diabetes, early onset heart disease; all problems typically\u00a0associated with an older population.<\/p>\n<p><span style=\"text-decoration: underline;\">A TWO (OR THREE) TIERED SYSTEM<\/span><\/p>\n<ul>\n<li>The \u201cfinancially advantaged\u201d will always be able to receive\u00a0care, most likely from a concierge physicians\u2019 service.\u00a0Painfully, 40% of physicians 55 years of age and older are\u00a0opting out of Medicare participation and this number is\u00a0growing. This is very bad news for aging Baby Boomers\u00a0who are bound to need increased health care.<\/li>\n<li>The \u201cfinancially disadvantaged\u201d will receive care via Federal\u00a0programs implemented at the State and Local levels.<\/li>\n<li>Sadly, the majority of Americans will fall into a system of\u00a0quasi-rationing.
